Our office schedules by appointment, usually being able to work emergency situations in within a timely manner. We recognize the value of your time and desire to serve you efficiently. Every effort is made to see our scheduled patients on time.
If you are unable to keep your scheduled appointment, please call us at least 24 hours in advance. Also, please call if you know you are going to be late. This courtesy allows us to accommodate other patients.

On your first visit to our office, please plan to:
- Come at least 15 minutes early so you’ll have time to fill out the necessary forms.
- Bring a list of all medications you are presently taking, any X-Rays, medical records, referral forms and diagnostic testing results that you have.

If your insurance requires you to have a referral, it is your responsibility to bring it to your appointment. Please have your referral ready at the time of check in. If you have had X-Rays, MRI, or CT Scans taken, it is very important that you bring them to the appointment.
Payment of coinsurance or co-payment amounts are collected at time service is rendered. We will bill your insurance for the portion they pay per your plan benefits. For your convenience, we accept cash, checks, and credit cards (Visa, MasterCard and Discover). When an emergency situation requires follow-up care in our facility, we will consider payment arrangements for balances over $500.00. Payment arrangements will be approved by our Financial Department, and will not be offered for elective procedures. Co-payments are collected at check-in and if you come unprepared and unable to pay your insurance co-payment, you appointment will be rescheduled. It is a good idea to arrange your appointments around your payday schedule – please consider this when you call to make your appointment because we expect you to come prepared to pay your copays, coinsurance amounts and unmet deductible amounts on the day you receive service. Our computer system holds most insurance plans contract rates and we are immediately able to calculate the amount of your co-payments and coinsurance amounts, surgery cost shares and deductibles. Most physician offices do not have this capability and would instead send you a statement after billing your insurance; we do not. We collect at the time services are provided.
If your minor child is a patient, please do not send them alone. Although teenaged children are capable of coming alone, we ask that the responsible adult accompany them to be available to take of the medical and financial issues that will arise.
Minors coming in without the responsible adult will have their appointment rescheduled to a time when you can accompany them. It is unlawful for us to treat a minor (under 18 years of age) without the consent of a parent of legal guardian. After receiving your care, we ask that you watch for your explanation of benefits from your insurance plan. Patients usually receive EOB prior to the physician’s office receiving the insurance payment. If you have not received one within 45 days of service, please contact them and inquire as to any reasons of non-payment.
If you are on a signed payment arrangement plan and a problem arises that effects your ability to continue making timely payments, you must contact our Financial Department immediately for assistance in the management of your account. Communication is key. It helps us avoid having to place you account with our collection agency. Once in collections, additional interest and fees will accrue and the collection agency does report balances to TRW. In the event of your account being placed with the collection agency, no further appointments for new problems will be made until prior debt is resolved.
A fee of $25.00 will be charged to your account for returned checks. From that point on, we will only accept cash, money orders, or credit cards for any payments currently due on you account of for future medical services rendered.
Medical Record Requests
If the need arises where you would need medical records, we require 5-7 days notice to process your request. There is a copy-of-records fee and will be quoted to you upon request. Medical record requests must be in writing with a signed medical release. Our Medical Records Department will be happy to fill your request once you meet these simple requirements
To comply with federal regulations (HIPAA) this office has established procedures to make your identity and medical records secure. Our only use of your personal information is for billing purposes and for proper medical treatment. We must have on record people, if any that we may release information such as husband, wife, daughter, son, etc.
X-Ray Copy Requests
If you are taking medications prescribed by our physicians and you need a refill, please call your pharmacy first. Your pharmacy will fax over the refill request, per established guidelines. We ask that you allow 48 hours for processing your request. Make sure you call in your refill while you still have a few days of medication remaining to get you through the 48 hour period.

Occasionally patients require a copy of their X-Rays. When this need arises, please contact our office and allow 48 hours for processing your request. There will be a fee of $10.00 per film payable when you pick up your x-rays.
